The forms are in Adobe Acrobat format. You can download the
FREE reader from:

Please note that you can fill out and print these forms with
either the FREE Adobe Reader software or the commercial Adobe
Acrobat software (4.0 or higher). However, if you wish to
electronically save the information you fill in or save
"default" information in some of the fields, you must have the
commercial Adobe Acrobat software. The free Adobe Acrobat Reader
software will not allow you to save forms data.

Forms should be printed out in triplicate. Turn two of the
copies in to Printing Services and keep one copy for your records. |

To prevent the question marks from printing on your forms,
make sure the "Annotations" or "comments" check box is NOT checked in your print
menu. |
